JOB SUMMARY:

The LPN provides clinical duties, evaluates and monitors the health and safety of the residents, takes care of medication administration, provides first aid tasks as needed. The LPN works as an educator to the residents and their families, may provide group facilitation, collaborates with the treatment team, and works closely with the Psychiatrist providing scribe duties. The LPN is also responsible for performing clerical functions and documenting related to record keeping and general office functions in a timely manner.

EDUCATION REQUIREMENTS/QUALIFICATIONS:

LPN Licensure

CPR and First Aid training for Healthcare Providers

Minimum 21 years of age

Possess an understanding of adolescent mental health behaviors and issues.

Effective human relations skills when dealing with staff, residents and families.

Effective written and verbal communication skills. A working knowledge and experience with Windows environment and MS office professional.

Maintain a good driving record and possess a valid driver’s license.

JOB RESPONSIBILITIES

Administer medications and document therapeutic effects

Take vital signs (temperature, blood pressure, pulse and respiration)

Work closely with all health care providers that participate in the residents care and treatment staff to ensure the appropriate delivery of physical and mental health care services.

Maintain medical records in accordance with licensing and HIPAA standards on Electronic Medical Record systems.

Monitor, evaluate and assess the clients referred from the treatment teams for real or potential, acute and/or chronic health problems, with an emphasis on wellness.

Works closely with Psychiatric provider and assisting with documentation/scribe duties.

Ability to maintain organization while being flexible with responsibilities.

Provide education and or group facilitation as needed.
